    Does anyone have examples of protocols and procedures they are using as far as their policy on which things should be entered as actions vs. which things shouldn’t.

    The rule of thumb is to enter as much as you can in actions, attributes, etc. That way, you can query on it when you need it.  If you stick it in Notes, you cannot easily retrieve the info.  We have an SOP manual for RE data entry, but it's basically a cut-paste piece from the RE User Guides.
