    Contact/Action Report Form

    I am trying to create a Contact Form to be used by our Major Gift Officers in conjunction with Microsoft Outlook, maybe create a form in Microsoft, and have the Officers fill them out after each visit. Does anyone have any experience or any Contact Forms they would like to share? Please contact me at [Email Removed]

    I have always created one from scratch.  (Around here we call it a "peach sheet" cause it's copied on peach-colored paper!)  I suggest you create a form with questions/lines that are closely alligned with Action and Prospect/Proposal fields in RE (assuming that you use RE).  That way, when the data-entry staff (or whomever) enter the info into RE you've made their job easier.
