Has anyone created a cultivation form or card (hardcopy) that board members and other non-RE users can utilize to submit new constituents including classifications such as sponsor, donor, invitee, etc.  I know I can make something from scratch but if someone has a successful example that they would be willing to share it would be a great help.  I am a bit overloaded at the moment with setup. We are new to RE.
