I am hoping to get a few more schools to request a check box on the Medical Tab for Dental Form Received. In Pennsylvania, Dental forms are requred for certain grade levels. We can add an attribute, but it would be better to have the Dental form Recieved check box directly under the Medical Form received and Immunization Form Recieved options.

If you would like to track these items quickly through a query, ask for the Dental form recieved Check box be added to the Medical tab!!!!

Are you talking about Knowledgebase BB454574 ?

As a temporary workaround - we added "Dental Form" as an immunization type.  Not quite right - but it keeps it in the medical tab on the record.
